“The partnership was fostered following two evening education sessions that were held in Nottingham.  GPs from across the four CCGs which cover the Nottinghamshire footprint (of which Rushcliffe CCG is one) were invited to the educational sessions delivered by a local Consultant Cardiologist and a Cardiology GPwSI around the subject of Heart Failure with a focus on Left Ventricular Systolic Dysfunction.  At these meeting GPs were offered the opportunity to host the Primary Care Heart Failure (PCHF) Service.   

As a CCG we wanted to ensure equality for all our patients but we were a little concerned there could be added strain to the local healthcare economy.  We were pleasantly surprised to find that there was only a limited impact on existing services, even after all the practices within our CCG had undertaken the PCHF Service.  The majority of the patients who were seen required medicine optimisation with a much smaller number than we expected requiring further diagnostics or secondary care intervention.  

The outcome of running the service across the whole CCG was that we ensured equity for all our patients with those deemed appropriate being seen by a local Consultant Cardiologist within their own GP practice and education for GPs during the PCHF clinics.

I would recommend this service as it achieves evidence based best practice.”
